Usage Note
Ausrenken means to dislocate a joint — sich den Arm ausrenken = to dislocate one's arm. Often used hyperbolically: sich fast den Hals ausrenken = to crane one's neck, almost dislocating it.

Natural Translation
He twisted around so clumsily that he almost dislocated his shoulder.
Literal Translation
He pulled himself so clumsily around that he almost the shoulder dislocated.
